As a Mosaic at Home Coordinator, you will play a vital role in ensuring individuals receive personalized support and have a voice in their own lives. What You'll Do: - Provide Direct Support: You will provide direct care and support, averaging 15-20 hours per week. This includes assisting individuals with daily living, facilitating community integration, and attending occasional evening and weekend events (approximately one per month). - Case Management & Monitoring: Manage a caseload of 22 individuals in Independent Living and Intermittent Services. You will complete monthly home visits and oversee the services delivered by their Direct Care Workers (DCWs) to ensure compliance with contracts and individual support plans. - Coordinate Care: Schedule and attend client medical appointments, ensuring all health needs are addressed. - Develop and Maintain Documentation: Write and regularly update individual support plans, programs, and assessments. You will also monitor DCW documentation to ensure accuracy and compliance with services provided. - Participate in On-Call Rotation: Join a rotating on-call schedule with other team members. You will be on-call for one week at a time, approximately once every six weeks, to provide support for urgent situations.
